<\/span>Step 3 \u2013 Gazette Notification<\/strong><\/span><\/h3>\n\n\n\nNow for getting your name published in the Official Gazette of\nyour state, for example: \u201cThe Delhi Gazette\u201d, you need to do the following\nthings:
\n1. Visit your local Government Press or the department that mobilizes Gazette\nnotification.<\/p>\n\n\n\n
2. Get 2 copies of \u201cDeed Changing Surname Form\u201d. (Fill this form\ncarefully as per the instructions provided therein)<\/p>\n\n\n\n
3. If possible get the phone number, or the mobile number of the\nperson of that department who can help you for Gazette Notification.<\/p>\n\n\n\n
Verification<\/strong><\/p>\n\n\n\nYou need\nto send the following documents to the Secretary, Department of Commerce &\nTransport (Commerce), _________________ (State). <\/p>\n\n\n\n
- Letter to the Secretary, requesting\nhim to allow you to change your name. (Click here to view sample letter<\/em>)<\/li>
- The \u201cDeed Changing Surname Form\u201d<\/li>
- 2 True copies of the newspapers in\nwhich your name change statement is published.<\/li>
- Attested copy of the Name Change\nAffidavit.<\/li>
- A print out of the statement in plain\npaper that you need to publish in your official Gazette. (Generally Same as\nyour Newspaper Statement)<\/li>
- A Copy of your current identity like\nyour Voter Identity Card, PAN Card, Birth Certificate, School or College\nCertificates and etc.<\/li><\/ol>\n\n\n\n

- Deposit the required amount for the\npublication. Here the amount depends on the kind of publication you want to go\nto (General and Extraordinary.). If you go for the extraordinary one, you can\nget the copies of the publication within a week or two.<\/li><\/ol>\n\n\n\n
